2011년 춘계 대한임상탈모학회 개최에 즈음하여
안녕하십니까?
2010년 10월부터 대전,서울,부산에서 임상탈모학회세미나가 열렸으며.마침내 2011년 춘계학술대회가 열리게 되었습니다.
사실 우리나라질병의 최고빈도(인구의 20%:1천만)인 탈모증, 다른의학은 급속히 발전했지만 현재 탈모치료는 아시다시피 걸음마수준입니다.
Hair Follicle은 Bone Marrow나 Tumor Cell만큼 metabolism이 빠르고 주위의 모든요인에
가장 sensitive 합니다.
탈모의학교육과정,탈모교과서,탈모논문도 거의 없으며 치료에 대한 원칙과 기준도 없이 경험의학적 측면 즉 남성호르몬 억제제,두피메조세라피,topical minoxidil제제에 거의 의존하는듯 합니다.
탈모가 이정도로 해결된다면 얼마나 좋겟습니까?
아시다시피 의학은 evidence위주의 최고의 과학입니다.
탈모치료는 복잡한 우리몸의 종합판입니다.
탈모는 medical condition의 하나의 symptom입니다.
이 medical condition을 해결해야 비로소 탈모가 해결되는 것입니다.
단순 두피에서의 관리와 치료는 치료동안 일시적으로 모발을 굵게할 뿐입니다.
의사의 전문치료는 가늘어진 모발을 굵게 하면서 miniaturized된 모발이 빠지고 난 뒤
즉 kenogen상태의 위축된 hair follicle이 빨리 올라와 건강한 hair cycle을 밟을 수 있도록 해야합니다.
예를 들어 Hypothalamic-Pituitary-Adrenal,Gonadal,Thyroid Axis는 Brain-Hair Follicle Axis에도 똑같이 작용합니다.그만큼 복잡하다는 사실입니다.그리고 남성형탈모는 정신적인 문제외에도 요즈음 급증하고 있는 coronary heart disease,prostate cancer를 정상인보다 훨씬 많이 동반하고 있으며 그 외 인슐린저항성,고혈압,고지혈증,다낭성난소증후군,당뇨등도 빈도가 높습니다.
그리고 수많은 암환자의 화학요법이나 방사선치료후 오는 탈모의 빠른치료는 환자에게 암예후에 도움을 주고 탈모환자(특히 여성에서)의 자존심을 살리는 길입니다 .그리고 보면 암전문병원에서 암환자의 탈모에 대해 얼마나 무관심하고 무지하다는 것을 알 수 있습니다.
그리고 요즈음 크게 증가하고 있는 소아 & 청소년 탈모치료도 거의 손을 놓고 있는 상태입니다.
탈모는 기초의학처럼 근본을 충분히 공부 & 연구하고난 뒤 차근차근 풀어나가야 환자의 전체 몸도 좋아지면서 해결될 수 있습니다,
이런 과정을 간과하고 빨리 빨리 치료법(특히 두피치료)에만 매달리는 것은 사상누각일 뿐입니다.탈모는 다양한 원인이 있고 환자마다 치료가 다르기 때문에 일반피부치료나 성형수술처럼 tip이 있을 수 없습니다.
이번 춘계 대한임상탈모학회 학술대회에서는 단 하루로는 정말 부족한 시간이지만 탈모치료에 대한 방향을 올바르게 잡아 대한민국 탈모학발전과 탈모환자의 고통이 하루 빨리 해결되기를 바랍니다.
